Safe2HelpNE School Participation Form
Safe2HelpNE is an anonymous reporting system for students, school staff, and parents to share information about concerning behavior so an intervention can happen before the unthinkable happens.
This form will register your district and indicate the five core threat assessment team members who will receive communication from the Safe2HelpNE communications center when a report for a school in your district is received.
Before completing this form, please ensure that you have the following information for each core team member:
- Name
- Role / Job Title
- Primary phone number with text capabilities
- Secondary phone number to be reached e-mail address
- The rank order in which the members would be called if necessary
- Date of Basic Threat Assessment Training
- Contact information for person in district who monitors social media
